FS Tennis Covid Screening and Indemnity forms

Dear player
Please find attached the forms that need to be completed before the start of play. The screening form needs to be completed everyday before 8:00 am. Indemnity form needs to filled in once off before the start of play on Friday 21 August at 12:00pm.

Please take note off the following as well:
Players please take note of the following;
⁃ All players are to report to the gate of the court where they will be playing their match (as scheduled on the order of play) 15 minutes (no earlier) prior to the scheduled match time. Eg : all 12:00 matches to report at 11:45 at the respective gates and not at the tournament desk.
⁃ An official will screen each player before the start of each match at the entrances to the respective courts where balls will be handed out.
⁃ Please make sure that both forms (Free State Screening Questionnaire and FS Tennis Indemnity) before the start of play on Friday. Links are available on the tournament site at the following link : https://tsa.sportyhq.com/x72in If there are forms outstanding, players will not be allowed to play.
⁃ Both parents can accompany their child, but have to remain in their vehicle at ALL TIMES. NO EXCEPTIONS.
⁃ Coaches that hold a valid COVID-19 Coaching licence will be allowed on the venue. Masks have to be worn at all times and coaches have to be screened by a tournament official.
⁃ Masks have to be worn at all times by ALL players, before entering the court to start your match.
⁃ Players have to leave the venue upon completion of their matches.
